The
Minnesota Consent Form to Release Health Information was developed
by the Minnesota Department of Health as required by the Minnesota
Health Records Act of 2007 (Minnesota Statutes, section 144.291
- 144.298). The form meets Minnesota and federal laws related to
the release of medical records including the Minnesota Data Practices
Act (Minnesota Statutes, Chapter 13), the HIPAA Privacy Rules (45
CFR Parts 160, 162, and 164) and Confidentiality of Alcohol and
Drug Abuse Patient Records (42 CFR Part 2). If this form is properly
completed, it must be accepted by a Minnesota health care provider.
